Unless your arrival requires use of the Emergency Entrance, come to the Main Hospital Entrance on Cree Drive.  The Admissions Office is conveniently located on the first floor, just inside the Main Entrance.  Admissions hours are 7:00 a.m. to 5:00 p.m., Monday through Friday.  During your registration for medical services, personal and medical information will be collected.  You will be asked to sign an Admission and Consent to Medical Treatment form.  All information will be handled confidentially.  Please bring your medical insurance card, photo ID, and any physician referral, if applicable.
