The Extended Care Unit at Lock Haven Hospital is a 120-bed long term care facility. We are currently JCAHO accredited, attaining a score of 98 out of 100 on our last survey, and licensed by the Pennsylvania Department of Health. We are Medicare and Medicaid certified and we accept most private insurances. We provide both skilled nursing care and long term nursing home care. The facility has three floors, with residents living on both the first and second floors and with an outside, covered patio on the ground floor.

We are attached to the hospital and many of the local physicians care for residents in the Extended Care Unit. Our residents receive individualized services from our Physical, Occupational, Speech and Recreational Therapy departments. Our progressive Restorative Nursing Program is instrumental in supporting our residents’ ability to maintain their independence. Each resident has an individualized plan of care developed with the resident’s involvement and coordinated by a Registered Nurse. There are Registered Nurses, Licensed Practical Nurses and Nursing Assistants on all three shifts.

Residents are involved in the planning of the Activities Program that provides daily group and individual activities with outside trips at least four times a year. We employ a full time social worker to assist residents and families and who coordinates our Resident/Family Council.

Menus are planned by a registered dietitian and snacks are available at all times. Barber/beautician services are provided weekly.
